Description

In the Town of Cazenovia, the Town Clerk wears many hats and is charged with tasks ranging from Registrar of Vital Statistics to Recording Secretary and Records Manager.

Responsibilities:

The Town Clerk issues birth and death certificates, birth and death transcripts and burial transit permits and is responsible for keeping a permanent record of all births, deaths and marriages that occur within the town since 1884. The Clerk acts as custodian of all permanent town records and maintains a record of adopted ordinances and local laws, oaths of office, resignations, proofs of publication and posting, annual budgets, assessment rolls and fiscal reports.

As Recording Secretary, the Town Clerk keeps a permanent record of all Town Board meetings and public hearings. The Deputy Town Clerk keeps a record of Planning Board and Zoning Board of Appeals meetings. The Town Clerk also issues many New York State licenses and permits including marriage, dog, and handicap parking.

Where to Apply: Town of Cazenovia Municipal Office at The Gothic Cottage at 7 Albany Street in the village of Cazenovia.  

Requirements:  Everyone who intends to get married in New York State must obtain a marriage license from a Town or City Clerk.  The license is valid for use anywhere in New York State (outside New York City) for a period of sixty (60) days.  Once the license is issued there is a 24 hour waiting period before the couple can marry.

  • The couple must meet the following upon filing for a marriage license:
  • Both parties to the marriage must be present.
  • Be at least eighteen (18) years of age or older
  • Both parties must present photo identification (i.e. Driver's License or Passport)
  • Both parties must present certified copies of their birth certificates
  • Both parties must present their Social Security cards or know their Social Security numbers
  • If applicable, both parties must present certified copies of divorce decrees for any prior marriage(s)

A NYS marriage license application asks basic information such as mother's maiden name, your city/town of birth date, address, and country of where your parents were born. 

*Costs:  The application fee ($40.00) includes the issuance of a Certificate of Marriage Registration.  This certificate is automatically sent by our office to the applicants within fifteen (15) calendar days after the completed license is returned by the officiant (person who performs the marriage ceremony).  It serves as notice that a record of the marriage is on file.  Couples who do not receive a Certificate of Marriage Registration within four (4) weeks of the wedding should contact the Town Clerk.

Who is eligible to obtain a birth certificate copy?

  • The person named on the birth certificate (if 18 years of age or older)
  • A parent of the person named on the birth certificate (requesting parent's name must be on birth certificate)
  • A person with a New York State Court Order

Identification Requirements (application must be submitted with copies of either A or B):
A.  One (1) of the following forms of valid photo-ID:

  • Drivers Licence
  • State issued non-driver photo-ID card
  • Passport U.S.
  • Military issued photo-ID

B.  Two (2) of the following showing the applicant's name and address:

  • Utility or telephone bill
  • Letter from a government agency dated within the last six (6) months

Who is eligible to obtain a death certificate copy?

  1. The spouse, parent, sibling or child of the deceased
  2. Other persons who have a: 
  • documented lawful right or claim
  • documented medical need
  • New York State Court Order issued on a showing of necessity

What is a lawful right or claim?
If the applicant is not the spouse, parent, sibling or child of the decendent, a lawful right or claim must be documented.  An example of a lawful right or claim would be a death record needed by the applicant to claim a benefit.  Documention would consist of an official letter from the agency verifying that to process the claim they require from the applicant a copy of the requested death record.

Identification Requirements (application must be submitted with copies of either A or B):

Marriage records are available from the Town or City Clerk of the municipality where the couple applied for the marriage license.

Who is eligible to obtain a marriage certificate copy?

  1. The bridge or groom
  2. Other persons who have a:
  • documented judicial or other proper purpose
  • New York State Court Order

Application for copy of Marriage Record

No information shall be released from a Record of Birth unless:

  • the record has been on file for at least 75 years and
  • the person to whom the record relates is known to the applicant to be deceased

No information shall be released from a Record of Death unless:

  • the record has been on file for at least 50 years

No information shall be released from a Record of Marriage unless:

  • the record has been on file for at least 50 years and 
  • the parties to the marriage are known to the applicant to be deceased

The time periods specified above are waived if the applicant is a descendant or has been designated to act on behalf of a descendant of the person whose record is being requested.  The applicant shall provide documentation of descendancy prior to the release of information in those instances where a waiver of the waiting period is requested.  A party acting on behave of a descendant shall further provide documentation that the descendant authorized the party to make such an application.

Where to Apply:

To apply for the parking permit:

(a)  Fill out Part 1 of the application.  If you have a PERMANENT DISABILITY, have a Medical Doctor, Doctor of Osteopathy, Physician Assistant, Nurse Practitioner, Doctor of Podiatric Medicine (for disabilities related to the foot) or Optometrist (for blindness) fill out the "permanent disability" Medical Certification section in Part 2 of the application form and return the form to you.  If you have a TEMPORARY DISABILITY that requires the use of an assistive device, have a Medical Doctor or Doctor of Osteopathy fill out the "temporary disability" Medical Certification section in Part 2 of the application form and return the form to you.  NOTE:  If you have a permanent disability, the need for medical certification may be waived by the issuing agent if you have an obvious, visually-identifiable disability (such as the loss of a leg) OR if you already have license plates for persons with severe disabilities.   

(b)  You must bring your completed application to the Town of Cazenovia Municipal Offices at 7 Albany Street in the Village of Cazenovia.  DO NOT SEND YOUR APPLICATION TO THE DEPARTMENT OF MOTOR VEHICLES (DMV) - DMV DOES NOT ISSUE PARKING PERMITS.   

Requirements:

You are eligible for a special parking permit if you are a New York State resident who has one or more severe disabilities that impair your mobility.  For a description of these disabilities see Part 2 of the attached application.  A parking permit may be issued for either a permanent or temporary disability.  Permits are issuedin the name of the person with the disability.  You do not have to be a driver, or the registered owner of a vehicle, to get a parking permit.  Children of any age who have a disability are eligible for permits, as are persons who are legally blind.   

 

According to the NYS Vehicle and Traffic Law and the Penal Law, it is a crime (a felony or misdemeanor) to make a false statement or to provide false information on an application for a parking permit for a person with severe disabilities.  This crime is punishable by a fine, impisonment, or both.  If this crime involves an application for a parking permit, the crime may also result in liability for payment of a civil penalty of $250 - $1,000.
